Cardiology Care with a Geriatric Medicine Perspective
Approaching Every Patient with Success in Mind.
Starting as early as the late forties, the human heart starts to undergo changes which accelerate as we get older. Many of these changes not only impact the progression of medical conditions, but can also change the response to treatments. We understand this very well and avoid using a cookie-cutter approach. Instead we use systematic and personalized care plans based on our experience as both cardiologists and geriatric medicine experts (figure A).
Our team of highly trained and experienced Doctors and Advanced Practice Registered Nurses (Nurse Practitioners) and our supporting cast of excellent nurses, technicians, and administrative staff are eager and always ready to help.
Our practice uniquely prioritizes cardiology care of patients in skilled nursing facilities (SNF) and residents of assisted living facilities (ALFs). A separate division of our practice focuses on care of hospitalized patients and those in long-term acute care (LTAC). We also maintain an active office practice for follow up after discharge.
We are highly collaborative practice and co-manage patients with other cardiologists, specialists, and primary care providers for a true team-based care approach.
