When patients with multiple chronic conditions get short of breath it’s not always clear which way they will go. Some will get better while others deteriorate quickly. Premature hospital transfer is both time consuming and very disruptive to both patient and the facility. Worse, the resident may get to the hospital only to be sent right back after a few hours. We created program to help. 

Virtual Acute Care (VAC) is a unique program which leverages the expertise of our doctors and cardiology nurse specialists as well as the latest technology to assess, treat, and monitor residents with hospital-level systems right at their SNF or ALF.   

The key to the program’s success is appropriate patient selection. When consulted, we send a provider to evaluate the resident face to face and help the medical director decide whether transfer or VAC is most appropriate. If the resident is transferred we will participate in their care at the hospital which provides continuity. And if they are discharged back to the facility we initiate the VAC program and keep a close eye on them until symptoms improve.

Residents on the VAC program are seen on rounds by one of our providers as frequently as needed-sometimes daily. VAC patients are also monitored 24/7 by an on-call cardiology specialist as well as by our state-of-the-art remote monitoring devices with artificial intelligence support to detect abnormal heart rhythms and other changes.
